Making An Appointment
Dr. Churney & Dr. Adams are available for consultation, evaluation and procedures by appointment only. The office has a wide range of early morning hours to accommodate a variety of schedules.

Appointments can be made by calling 727-799-6733 or use this Appointment Request Form.

When you make your first appointment at the Churney Periodontal, you have the opportunity to review all of the information and download forms in the Patients Forms & Documents section of our website. You may want to read over them, fill them out to bring to your appointment, or have a friend go over them with you in advance. It is not a requirement that you bring your forms already filled out.

We will always have patient forms available to you and a staff member to go over the information with you if you have questions or need assistance with the forms.

If you are more than 15 minutes late, your appointment may have to be rescheduled. First-time appointments are asked to arrive 15 minutes prior to their appointment to complete new patient information. Dr. Churney’s office requests 24-hour notice of appointment cancellation.

Canceling Appointments
If you have an appointment scheduled and you are unable to come, please call 727-799-6733 as soon as possible to cancel the appointment.

Please make this call before the appointment time as a courtesy to the other patients who may be waiting to see Dr. Churney or Dr. Adams.
